New Client Forms
All new clients should complete the following forms and bring them to your first therapy session. Child client intake forms are to be used for children under the age of 18. The Notice of Privacy Practices (HIPPA) form is included for your convenience, but does not have to be returned to the first session.
Initial Client Portal Setup

All clients are given an online portal account to complete initial intake questionnaires and access a portion of their electronic health records. Our Administrative staff will instruct you on how to set up your Patient Portal and access your client record online.

If you already have a login, simply click on the link below to get started. If you do not have a login, contact our Main Office and let our staff know you would like to be able to access your record online, talk to your provider, and complete your client measures electronically.
